B.3. Non-EEE national(1) with a degree from outside the EEA or from Switzerland
(1) You are a non-EEE national (EU, Iceland, Norway and Lichtenstein) or from Switzerland with a degree from outside the EEA (EU, Iceland, Norway and Liechtenstein) or from Switzerland:
In order to complete your application for registration on the Order of Pharmacists’ official register, please submit a copy of the following documents:
- a copy of your passport
- a copy of your original degree (or the provisional certificate issued by your university)
- a copy of the degree equivalence obtained either from the Wallonia-Brussels Federation (equi.sup@cfwb.be) or the Vlaamse Gemeenschap (naric@vlaanderen.be) according to your choice of language
- a copy of the authorisation to practise as a pharmacist issued by Royal Decree (Article 145 of the coordinated law of 10 May 2015 on practising professions in healthcare). If you have any questions about this, please contact the Public Health Service via info@sante.belgique.eu
- if you are still a student, a copy of the certificate issued by your university
- a copy extract of your judicial record, or, failing that, an equivalent document (dated within the last three months) issued by the city, municipality or the competent authority of the country of origin and/or the country of provenance
- a copy of the residence permit (regularly renewed)
- a copy of the work permit or professional card (regularly renewed)
- if you are registered in the Order in the country of origin, please submit proof of your deregistration from that Order by providing a copy of the document confirming deregistration. If you are not registered, a copy of a declaration stating that you have never been registered
- if you have already practised as a pharmacist in the country of origin or country of provenance, a certificate of professional ‘good conduct’ issued by the professional order (or competent authority) of the country of origin or country of provenance, and dated within the last three months.
This document must include a list of any professional or administrative measures or sanctions taken against the pharmacist, and any criminal sanctions relating to practising as a pharmacist in that country. - where applicable, a copy of the authorisation from the Minister of Public Health to provide clinical biology services (Royal Decree of 5 November 1964)
- if necessary, you may also submit other documents (such as certificates, contracts, etc.)
